These are the tokens I'd look to change out. High AC and damage reduction against attacks that have to hit is nonsynergistic. As people have mentioned, Retribution and high AC is nonsynergistic. If you were running solo, might be different, but not a lot of folks do that.
I'd rather rares than a number of the URs. Shaed, as much as I want to like it, would make more sense as +2 Cloak of Resistance. I'd rather Shirt of the Elder than Shirt of the Underdark. Damage boots or attribute bump over Dirtdazzy's. Ioun Stone to do melee damage should be easy to find. Lot of useful rings, Ring of Iron Will for instance. Belt of Thorns whichever belt that evens out your STR, Ogre Mage shouldn't go for that much.
Not terribly clear why Dungeonbane is on DEX. If part of the reason is to jack up AC, that's fine. AC becomes a lot more interesting the fewer number of people on the run. For NM, high 30s does mean unlikely to get hit.
Sure, can just ignore saves. Then, saves are dungeon dependent. But, not giving up much to get better saves. Can blow off Will for Fort or Ref if those seem more important. Alternatively, can load up on STR and damage in more slots.
Have two more bead slots. Bead of the Dire Bear is readily available. Can do whatever for third.
In general, if the idea is NM/Epic, more STR/to hit/dmg. VTD is a different animal as maybe good at sliding, but for VTD, would look for +18 to hit or higher and aim for +20 or more damage. This assumes interested in fitting some typical paradigm for what is desirable.
The damage reduction I'd look to run in a build with a much lower AC.
