Should Folio of Might have its Source changed from Treasure Chest to Appreciation? From what I know, Folio of Might was distributed in lieu of the 2020 adventure modules and was not included in treasure like the other folios.

Perhaps it should be both Treasure Chest and Appreciation? For what it's worth, the first Folio of Might I received came from a treasure chest at Gamehole Con. Although perhaps that was a one-off or mistake.

Should the Boots of the Four Winds, Kilt of Dungeonbane, and Supreme Ring of Elemental Command get the "Misc: Level Increase" tag? Of the Eldritch items, only the Rod and Skull appear to have it as of now.

Tokens that increase character level all by themselves get the Level Increase tag.

When a set bonus increases character level but none of the components by themselves increase character level, the individual tokens that make up that set do not get the Level Increase tag.

Rod of Seven Parts and Skull of Cavadar have the Level Increase tag because by themselves they bestow the ability to play a 5th level character when playing a sub class.

If one would like to debate the merits of this standard, one is welcome to open a new thread to do so.

Shouldn’t Totem of Heroism have “Note: +level effects do not stack” exception added to it?

Totem of Heroism now specifically states that +level effects do not stack.

Please be aware that that's only a courtesy reminder of a universal rule. A token without any kind of courtesy reminder is never capable of breaking any established rule unless the token or the TDb description specifically states otherwise.
